    Treosulfan, also known as L-threitol 1,4-bismethanesulfonate, is a bifunctional alkylating agent. It is a prodrug that undergoes non-enzymatic conversion in vivo to its active epoxide forms, which exert cytotoxic effects by cross-linking DNA strands, thereby inhibiting DNA synthesis and cell division. Primarily, treosulfan is used as a chemotherapeutic agent, particularly in the treatment of ovarian cancer. Its major application, however, is in the field of hematopoietic stem cell transplantation (HSCT). It is used as a conditioning agent prior to transplantation due to its potent myeloablative and immunosuppressive properties. Treosulfan-based conditioning regimens are considered an alternative to busulfan, especially for patients who may not tolerate high doses of busulfan or total body irradiation. The compound is characterized by its relatively favorable toxicity profile compared to some other alkylating agents, with potentially reduced incidences of severe organ toxicity, such as veno-occlusive disease of the liver. It is typically administered intravenously.
