    (1S,2R)-2-(octylamino)-1-[4-(propan-2-ylsulfanyl)phenyl]propan-1-ol, known as Sulodexide, is a highly purified glycosaminoglycan (GAG) preparation composed of a fast-moving heparin fraction (approximately 80%) and dermatan sulfate (approximately 20%). It is a mixture of polysaccharide chains with varying molecular weights, derived from porcine intestinal mucosa. The compound's structure is characterized by its sulfated glycosaminoglycan components, which are responsible for its biological activity. Sulodexide is primarily used as an active pharmaceutical ingredient (API) with antithrombotic, profibrinolytic, and antiatherosclerotic properties. Its main therapeutic applications are in the treatment of peripheral vascular diseases, such as chronic venous insufficiency and intermittent claudication, by improving microcirculation and reducing blood viscosity. It also demonstrates endothelial-protective effects and is investigated for potential use in diabetic nephropathy and other vascular complications. Its mechanism involves restoring the glycocalyx and modulating the activity of various factors in the coagulation cascade. As a prescription drug, it is administered via oral or intramuscular routes. Sulodexide is generally well-tolerated, with a lower risk of bleeding complications compared to standard heparin, due to its reduced anti-factor IIa (thrombin) activity while retaining anti-factor Xa activity.
