    Laninamivir is a potent and long-acting neuraminidase inhibitor. Its chemical structure, (4S,5R,6R)-5-acetamido-4-guanidino-6-((1R,2R)-2,3-dihydroxy-1-methoxypropyl)-5,6-dihydro-4H-pyran-2-carboxylic acid, is a cyclopentane derivative designed as a transition-state analogue of sialic acid, which confers high affinity for the viral neuraminidase enzyme. This compound is the active pharmaceutical ingredient (API) of the prodrug laninamivir octanoate (CS-8958, Inavir®). It is specifically indicated for the treatment and prophylaxis of influenza A and B virus infections. Its primary mechanism of action involves inhibiting the viral neuraminidase, an enzyme essential for the release of newly formed virions from infected host cells, thereby halting the spread of infection within the respiratory tract. A key distinguishing feature of laninamivir is its exceptionally long retention in the lungs following inhalation administration. This property allows for a single-dose treatment regimen, providing sustained antiviral activity over several days, which offers a significant advantage in patient compliance and outbreak management scenarios.
